TruNEST™ MODIFICATION REQUEST FORM

      This Modification Request is made and entered into pursuant to a certain TruNEST™ Software License Agreement datedby and betweenand MAGESTIC SYSTEMS, INC. ("Magestic") (the "Agreement"). This Modification Request shall be governed by, and become part of, the Agreement.

NOTE: This form is to be used for modifications that are not currently supported by a standard Magestic product or Maintenance and Warranty. If the requested modification can be satisfied by a standard product then that is what will be quoted.
